Duh..

After discovering that 22M and 44M RVM data partitions init okay, I 
realized that swap had been commented out of fstab, so the system 
was running without swap space  (only have 96M physical on this server)

So .. after adding swap, I was able to initialize at 130M rvm data 
partition.
