Brute force answer for Q1 and Q2:

[root_at_monamie root]# service venus.init stop
[root_at_monamie root]# rm -rf /usr/coda/venus.cache/*
[root_at_monamie root]# touch /usr/coda/venus.cache/INIT
[root_at_monamie root]# service venus.init start
Starting venus: done.
[root_at_monamie root]# clog coda_at_pbs.com
username: coda_at_pbs.com
Password: 
[root_at_monamie root]# cfs lv /coda/pbs.com
  Status of volume 0x7f000000 (2130706432) named "/"
